Please ensure that your file is kept up to date with the best phone numbers, email and home addresses, and emergency contact information. Please inform the receptionist of any changes to your personal information upon arrival at the clinic or call after any of the above has changed.
Our front staff are specially chosen and trained to help patients. We understand that there are many reasons why you may need to visit our office and we make every effort to make your visit as pleasant and comfortable as possible. In turn, we ask that your behaviour is respectful to our staff. There is a zero-tolerance for abuse of any kind and may lead to dismissal from the practice.
If you have any problems associated with your visit, please contact our manager in writing.
In order to provide you with the best possible care, a satisfactory doctor-patient relationship is necessary. If for any reason, this relationship becomes compromised, then it may be best for both parties to end the relationship. If this occurs then we will provide emergency medical care for 2 month and prescriptions for 90 days.
Our staff will give you a reminder notice 24 hours before your next appointment.
Please provide 24 hours cancellation notice otherwise 60$ fee will apply that needs to be settled before next appointment is booked.
If we have to cancel our clinical day we will try to provide as much as possible notice other than unforeseeable circumstances and situation.
It is your duty to book your next appointment within 7-10 days if your doctor ordered tests so results can be discussed.
It is your duty to follow the suggested treatment plan and follow-ups as requested by your provider.
The provider will not be held liable for end results of your treatment as results can be variable from person to person.
Payments are expected at the time of treatment in full otherwise treatment will be cancelled and 60$ fee will be applied for the provider’s lost time.
Failure to pay will result in cancellation of all future appointment and dismissal from the practice.
Dismissal from practice
Your provider has the authority to dismiss you from her practice for abusive behaviour,
Non-compliance with payments, recommendations made or when your provider feels that provider-patient relationship is not in the best interest of either or both parties.
Your provider will inform you about it personally followed by a letter from her practice.
Our Policy does no harm and your provider may choose to disagree with your expectations regarding your treatment outcomes.
